Abstract

The HPM for DEP in Phitsanulok Province consists of 5 main components: principles, objectives, content, processes, and evaluation. The process components consist of 6 sub-main steps: (1) Network brainstorming (2) Determining key focus points with the public and service recipients as the center (3) Health promotion for patients/service recipients (4) Health promotion in the community (5) Holistic Care, Humanized Health Care (6) Integrated care process. The results of the trial in 2023 showed that among the 0.6 % increase in the number of elderly people from 2022, the health promotion model for the elderly with dependency conditions was able to reduce the number of socially connected groups (ADL level 12- 20), homebound groups (ADL level 5-11), and bedridden groups (ADL level 0-4) from 2022 by 2.9%, 28.71%, and 2.04%, respectively. the model evaluation showed that (1) the level of satisfaction was at a good level, (2) the CIPP Model showed the Plan-DoCheck-Action learning cycle has been developed from 2019-2023 and trials have been carried out according to research and development methodologies and evaluation based on evaluation theory to create product innovations, and (3) the EFA of the model showed that the KMO value was equal to .962, which was close to 1. The Bartlett's Test (Approx. Chi-Square=8703.274, di=210, Sig=0.001) shows that the variables related to the health promotion pattern for the elderly with dependency are sufficiently correlated for factor analysis, which means that this model can be further utilized. Keywords: elderly with dependency, long-term care for the elderly, promoting the health of the elderly
