Inclisiran helped more high-risk people reach cholesterol goals
At 360 days, 89.5% reached cholesterol goals with inclisiran plus optimized therapy, versus 62.2% with optimized therapy plus placebo.
Double-blind, placebo-controlled
Limit: Cholesterol targets, rather than heart attacks or lifespan, were assessed

The randomized trial enrolled 1,770 people with high cholesterol and high or very high risk of heart and blood vessel disease, already taking their highest tolerated doses of statins, cholesterol-lowering drugs. Participants received inclisiran sodium 300 mg (284 mg inclisiran) or placebo on days 1, 90 and 270, with other cholesterol drugs individually adjusted through day 360. Everyone switched to rosuvastatin, a statin, at randomization. Inclisiran improved attainment of targets for low-density lipoprotein cholesterol, often called “bad” cholesterol, with less need to intensify other drugs.
Muscle-related adverse events with inclisiran versus placebo occurred in 33.3% versus 60.5% taking no rosuvastatin; 10.8% versus 35.9% taking up to 10 mg; and 13.5% versus 25.5% taking over 10 to 20 mg. Among those taking no statin, with or without the cholesterol drugs ezetimibe or bempedoic acid, rates were 24.0% versus 57.9%.
Why it matters
Cardiovascular disease is an important concern in aging. This study addressed how to pursue recommended cholesterol targets while limiting the need to intensify other medicines.
Caveats
This exploratory analysis, reported in a conference abstract, assessed cholesterol targets rather than establishing effects on heart attacks or lifespan. The findings concern high-risk people already taking statins, not people at low cardiovascular risk.
