Abstract

Introduction and Objective: Clinical trials show tirzepatide improves obstructive sleep apnea (OSA) severity, but real-world data are limited. This analysis evaluates changes in patient-reported OSA symptoms after treatment with tirzepatide. Methods: We extracted EHR data from a sample of 5372 patients with overweight or obesity and a history of OSA who received tirzepatide via telehealth for at least 28 days. In June 2025, patients received a survey assessing perceived changes in sleep-related outcomes, using responses adapted from the Patient Global Impression of Change scale, measured on a 7-point Likert scale ranging from “much worse” to “much improved.” Responses were dichotomized as improved vs no change or worsened. Proportions were compared using χ² tests with corrections for multiple comparisons. Results: A total of 199 patients responded to the survey and 181 provided data suitable for analysis. Median age was 48 years (IQR, 41-56) and median pretreatment BMI was 37.2 kg/m² (IQR, 32.7-41.1); 53.0% were female (n = 96) and 46.4% male (n = 84). Median treatment duration was 87 days (IQR, 54-127). A majority (86.7%; n = 157; 95% CI, 80.7%-91.2%; χ² = 97.73; P < .001) reported improvements in at least one sleep-related outcome, and 77.3% (n = 140; 95% CI, 70.4%-83.1%; χ² = 54.15; P < .001) reported improvements in multiple areas. Improvements in energy levels were reported by 72.4% (n = 131; 95% CI, 65.2%-78.6%; χ² = 36.25; P < .001), sleep quality by 59.7% (n = 108; 95% CI, 52.1%-66.8%; χ² = 6.77; P = .028), daytime sleepiness by 60.2% (n = 109; 95% CI, 52.7%-67.3%; χ² = 7.56; P = .024), and snoring by 59.1% (n = 107; 95% CI, 51.6%-66.3%; χ² = 6.02; P = .028). Conclusion: In this real-world cohort of adults with overweight or obesity and a history of OSA receiving tirzepatide treatment on a telehealth platform, the majority reported perceived improvements in multiple sleep-related symptoms. Findings complement trial evidence by demonstrating that sleep-related benefits observed in controlled settings may also extend to routine clinical care, including telehealth. Disclosure: L.
