Abstract

Introduction and Objective: Metabolic memory describes lasting effects of early glycemic control on long-term outcomes in diabetes. While seen in clinical trials, real-world evidence remains limited, mainly in South Asia. We investigated whether achieving early normoglycemia (HbA1c <6.5% in first 5 years post-diagnosis) provides lasting protection against complications independent of subsequent glycemic control in a community diabetes cohort. Methods: Retrospective analysis of approximately 400 T2DM patients with ≥5 years duration from diabetes screening camps. Patients stratified by early glycemic control: “Early Achievers”(documented HbA1c <6.5% within first 5 years, n=89) versus “Late/Non-Achievers”(never achieved <6.5% in first 5 years, n=311). Current complication burden, metabolic parameters, and diabetes duration assessed. Analysis adjusted for current age, BMI, and diabetes duration. Results: Despite similar current HbA1c (Early Achievers 7.2% vs Late/Non-Achievers 7.8%, p=0.08) and comparable diabetes duration (12.5 vs 11.8 years, p=0.42), Early Achievers demonstrated dramatically lower complication burden: multi-complication rate 28% vs 68% (p<0.001), representing 59% relative risk reduction. Specific complications: reduced bone density 58% vs 81% (p<0.01), neuropathy 32% vs 58% (p<0.001), dyslipidemia 48% vs 72% (p<0.001). Among patients with current suboptimal control (HbA1c 7-8%), Early Achievers still showed 52% lower multi-complication rate versus Late/Non-Achievers (35% vs 73%, p<0.001). Regression analysis confirmed early achievement as independent protective factor (OR 0.21, 95% CI 0.12-0.38) after adjusting for current HbA1c, duration, age, BMI. Conclusion: Achieving normoglycemia within first 5 years provides lasting 60% reduction in complication burden independent of subsequent control, demonstrating powerful metabolic memory in community-based type 2 diabetes. We need aggressive treatment immediately at diagnosis to capitalize on this critical window. Disclosure: A. Raha: None. S. Patil: None. A. Maheshwari: None.
