Florida adults—1,962,822 people who received their first Pfizer-BioNTech or Moderna COVID-19 vaccine dose between December 18, 2020, and August 31, 2021—were compared in a matched observational cohort study. Matching accounted for age group, sex, race, ethnicity, vaccination month, vaccination-site type and residential area.
Over 12 months, Pfizer vaccination was associated with higher mortality from all causes than Moderna vaccination. Pfizer recipients also had higher mortality from cardiovascular disease, COVID-19 and causes other than COVID-19. Differences in cardiovascular and COVID-19 mortality were primarily driven by adults aged 60 and older.
Why it matters
For older adults, the findings suggest that vaccine comparisons should consider both COVID-19 and other health outcomes.
Caveats
Vaccine assignment was not randomized, so differences between recipients could have influenced the comparison. Additional checks found no indication of meaningful unmeasured differences likely to change the results, but identified a possible small remaining imbalance among adults aged 60 and older.
