Abstract

Introduction and Objective: Current guidelines recommend individualized HbA1c targets but debate continues regarding optimal glycemic range. While intensive control (HbA1c <6%) often pursued, large trials suggest potential harm. We investigated the relationship between HbA1c levels and multi-system complications in community-based type 2 diabetes. Methods: Cross-sectional analysis of approximately 400 T2DM patients from community screening camps. Patients stratified by HbA1c: very tight control (<6%, n=68), optimal (6-7%, n=142), moderate (7-8%, n=118), poor (>8%, n=72). Comprehensive complication assessment: bone mineral density, peripheral neuropathy, dyslipidemia, multi-complication burden (≥2 complications). Comparisons adjusted for age, diabetes duration, BMI, and medication use patterns. Results: Multi-complication rates showed U-shaped distribution: HbA1c <6% (58%), 6-7% (42%), 7-8% (61%), >8% (74%). Optimal range (6-7%) had significantly lower complications versus very tight control <6% (42% vs 58%, p<0.05) despite better HbA1c. Patients with HbA1c <6% were older (62 vs 55 years, p<0.01), longer duration (13.2 vs 8.4 years, p<0.001), higher medication burden (2.8 vs 1.6 drugs, p<0.001), suggesting intensive treatment in frailer patients. Among HbA1c <6% group, 68% had neuropathy versus 45% in 6-7% group (p<0.01), 71% had bone loss versus 58% (p<0.05). Triglyceride levels paradoxically higher in <6% group (172 vs 142 mg/dL, p<0.01), indicating focus on glucose neglected lipid control. Conclusion: Patients with Diabetes having HbA1c 6-7% demonstrate lower complications burden than both tighter (<6%) and relaxed (>8%) control groups. Very tight control (<6%) associated with older age, longer duration, higher medication burden, and paradoxically elevated complications, suggesting potential harm from overtly aggressive glucose management. Findings support individualized targets with 6-7% as optimal range for most community patients Disclosure: A. Maheshwari: None. S. Patil: None. S. Singla: None. N. Verma: None. A. Tewari: None.
