Kidney filtration rose more with dapagliflozin after heart transplant

Estimated kidney filtration rose 2.0 mL/min/1.73 m² with dapagliflozin versus 0.3 with placebo from two weeks to 12 months after treatment began.

Kidney cross-sections beneath a small heart, with more fluid drops below the larger dark blue-green kidney than the smaller gray kidney.
Studied:
Randomized trial in 424 adult heart transplant recipients, 12 months, against placebo (Double-blind, placebo-controlled, multicenter)
What changed:
Estimated kidney filtration rose 1.7 mL/min/1.73 m² more with dapagliflozin than placebo after heart transplantation
The catch:
The outcome was estimated kidney filtration, not long-term kidney failure
In the record:
The 189th paper on dapagliflozin with results in people; 470 on record in all.
Why it’s aging research:
It studies an intervention with evidence of extending healthy life.

Adult heart transplant recipients—430 people at least one year after transplantation—entered a double-blind, randomized, placebo-controlled trial at six Northern European centers. They received dapagliflozin 10 mg or placebo once daily for 12 months. The analysis included 424 participants with a follow-up kidney measurement; their mean age was 53.

Dapagliflozin improved the rate of change in estimated kidney filtration from two weeks after treatment began to month 12. This estimate measures how well the kidneys filter blood. The between-group difference in filtration change was 1.7 mL/min/1.73 m². The groups had similar rates of adverse events, including infections, and the researchers reported no unexpected safety signals.

Why it matters

Progressive kidney disease is common after heart transplantation. Preserving kidney function is relevant to healthy aging in these recipients.

Caveats

The primary outcome tracked estimated kidney filtration, not long-term kidney failure. The findings concern adults at least one year after heart transplantation with estimated filtration of at least 25 mL/min/1.73 m².
