Abstract
Background: While amyloid correlates with white matter hyperintensities, tau distinctly impacts white matter microstructure through axonal injury from its prion‐like spread. Tau accumulation and white matter microstructural decline begin prior to clinical symptom onset, emphasizing their pivotal role in AD manifestation.
