Two cholesterol drugs showed no clear cardiovascular advantage
Over three years, cardiovascular events occurred in 8.5% of patients starting statin–ezetimibe therapy versus 7.9% starting a statin alone.

Our read
Interventions- Studied:
- Randomized trial in 3,048 patients with atherosclerotic cardiovascular disease, 3 years (Multicenter, registered NCT04626973)
- What changed:
- Cardiovascular events did not differ: 8.5% starting both drugs versus 7.9% starting a statin alone
- The catch:
- Some statin-only participants later added ezetimibe, so the trial compared starting regimens, not fixed treatments
- In the record:
- The 18th paper on Statin + ezetimibe with results in people; 27 on record in all. Its result on LDL cholesterol agrees with all 4 other findings on record.
- Why it’s aging research:
- It studies an intervention with evidence of extending healthy life.

In South Korea, 3,048 people with atherosclerotic cardiovascular disease, caused by fatty buildup in arteries, entered a randomized trial. Their average age was 64.4 years, and 638 were women. After assignment to a more intensive or conventional cholesterol target, 1,525 started two cholesterol-lowering drugs—a statin and ezetimibe—and 1,523 started a statin alone. Treatment was adjusted to reach their assigned target over three years.
The combined outcome included cardiovascular death, nonfatal heart attack or stroke, procedures to restore blood flow, and hospitalization for unstable angina, chest pain caused by reduced blood flow to the heart. Its frequency did not differ significantly between starting regimens. Starting both drugs produced lower levels of “bad” cholesterol under both target strategies. Reported safety outcomes were similar.
Why it matters
For cardiovascular disease, which becomes more common with age, the trial addressed how to choose starting drugs when treatment is adjusted toward cholesterol targets.
Caveats
The trial compared starting regimens, not fixed treatments: some participants starting a statin alone later added ezetimibe. The findings do not cover people without established cardiovascular disease; 20.9% of participants were women.
