Opportunistic salpingectomy for ovarian cancer prevention Number 12 - 2026

By da Silva AL, Cândido EB, Carvalho JP et al.

•Opportunistic salpingectomy (OS) is a biologically sound strategy for ovarian cancer prevention, as most high-grade serous carcinomas (HGSC) originate in the fimbrial epithelium of the fallopian tube. •Epidemiological data and meta-analyses consistently show a substantial risk reduction (approximately 40-80%) in tubo-ovarian carcinoma following salpingectomy. •The procedure is safe when performed during hysterectomy or benign gynecologic surgery, adding minimal operative time (10-16 minutes) without increasing perioperative complications. •Evidence shows that it is reliable to perform OS instead of tubal ligation or fimbrectomy for permanent contraception. •Current evidence confirms that OS does not impair short-term ovarian reserve or accelerate the onset of menopause. •Implementation requires structured counseling on the irreversible nature of the procedure and shared decision-making.
