A Recipe for Better Diabetes Management: The DASH4D Dietary Pattern

By Selvin E, Sullivan V, Yeh HC et al.

The 2026 American Diabetes Association "Standards of Care in Diabetes" includes extensive nutrition recommendations for people with diabetes but stops short of recommending a specific dietary pattern. Instead, the guidelines recommend that all patients be referred for individualized nutrition therapy. This approach appropriately acknowledges diverse patient needs; however, it is difficult for providers to deliver any form of individualized dietary guidance without a clear starting point to guide them. In this article we examine the tension between the needs for individualization and evidence-based nutritional standards in diabetes care. We argue that the DASH (Dietary Approaches to Stop Hypertension) dietary pattern stands out as being supported by decades of evidence from rigorous randomized clinical trials, meta-analyses, and large observational studies that have established its benefit for cardiovascular health. We summarize the recently completed DASH for Diabetes (DASH4D) trial, which demonstrated that a modified version of the original DASH eating plan, adapted for adults with diabetes, significantly reduced blood pressure and improved glycemic control in participants with type 2 diabetes and hypertension. We conclude that while individualized dietary counseling remains the standard of care, the DASH4D diet should be a recommended diet for patients with diabetes.
