Real-World Use of GLP-1 RA-Based Therapies for Weight Management in Ireland: A Cross-Sectional Study of Prescribing and Care Pathways, Costs, and Self-Reported Outcomes

By Catherine V George, Olga Cleary, Cathal O'Hara et al.

Background Glucagon-like peptide-1 receptor agonist (GLP-1 RA)-based therapies are increasingly used for weight management, yet evidence on their use and user experiences remains limited. This study aimed to describe prescribing and care pathways, costs, and self-reported outcomes among individuals using these therapies in Ireland and to explore implications for public health. Methods An exploratory cross-sectional online survey was conducted between May and July 2025. Adults (≥18 years) living in Ireland who were currently or previously using GLP-1 RA-based therapies for weight management were recruited through social media and professional networks. A 46-item questionnaire assessed sociodemographic characteristics, prescribing pathways, medication use, adherence, costs, behavioural changes, and support received. Descriptive statistics, Wilcoxon signed-rank tests, and multivariable logistic regression were performed using Stata/SE v18, with statistical significance set at p<0.05. Findings A total of 1,858 responses were analysed. Participants were predominantly women (97.5%), third level education (77.1%), and in paid employment (87.1%). GLP-1 RA-based therapies were most commonly prescribed by general practitioners (51.7%), followed by consultants (20.6%) and online licensed healthcare providers (20.7%). Median weight loss was 11 kg (IQR 7–17), and over 80% of participants reported satisfaction with treatment. Most participants reported monthly costs of €100–€300, with more than 10% exceeding €300. Approximately two-thirds received follow-up support, primarily focused on weight monitoring. Receipt of follow-up care and additional support was independently associated with better self-reported health and higher treatment satisfaction. Interpretation GLP-1 RA-based therapies are being used for weight management in Ireland through diverse, largely self-funded prescribing and care pathways. While participants reported high satisfaction and positive outcomes, variation in follow-up care, support provision, and affordability highlights important considerations for equitable and sustainable models of weight management care.
